People First Care Centre is seeking a dedicated and compassionate Care Coordinator to join our team in the United Arab Emirates. The Care Coordinator will play a vital role in ensuring that our clients receive the highest quality of care. You will be responsible for coordinating and managing client care plans, acting as a liaison between clients, their families, and healthcare providers, and ensuring that all care services are delivered efficiently and effectively. This role requires excellent organizational skills, empathy, and a strong commitment to client-centered care.

Key Responsibilities:

  • Client Care Coordination: Develop, implement, and manage individualized care plans for clients, ensuring that all aspects of care are delivered according to the client’s needs and preferences.
  • Communication: Serve as the primary point of contact for clients and their families, providing regular updates on care plans and addressing any concerns or questions.
  • Healthcare Collaboration: Coordinate with doctors, nurses, and other healthcare professionals to ensure seamless delivery of services and effective communication across the care team.
  • Scheduling and Planning: Manage and schedule appointments, follow-up visits, and any required therapies, ensuring that all necessary care is provided in a timely manner.
  • Documentation and Reporting: Maintain accurate and up-to-date records of client care, including progress notes, care plans, and any changes in the client’s condition or needs.
  • Advocacy and Support: Advocate for the client’s needs and preferences in all aspects of care, providing emotional support and guidance to both clients and their families.
  • Quality Assurance: Monitor the quality of care provided, identify any gaps or areas for improvement, and work with the care team to implement changes as needed.
  • Crisis Management: Respond to emergencies or changes in a client’s condition promptly and coordinate with the necessary healthcare professionals to ensure immediate care.
Qualifications:
  • Bachelor’s degree in healthcare management, social work, nursing, or a related field.
  • Previous experience in a care coordination or similar role within a healthcare or social services setting.
  • Strong organizational and time-management skills.
  • Excellent communication and interpersonal skills.
  • Empathetic and compassionate with a strong commitment to client-centered care.
  • Ability to work collaboratively within a multidisciplinary team.
  • Proficiency in using healthcare management software and electronic health records (EHR) systems.
  • Knowledge of local healthcare regulations and standards in the United Arab Emirates.
