Your responsibilities will include:
- Assessment and Support: Conducting comprehensive assessments of residents’ psychosocial needs and collaborating with interdisciplinary teams to develop individualized care plans that address these needs effectively.
- Counseling and Advocacy: Providing emotional support, counseling, and advocacy services to residents and their families, helping them navigate challenges and transitions associated with aging, illness, or disability.
- Resource Referral: Connecting residents and families with community resources, support services, and financial assistance programs to enhance their quality of life and promote independence and self-sufficiency.
- Care Coordination: Collaborating with internal and external stakeholders, including healthcare professionals, community agencies, and government organizations, to coordinate services and ensure continuity of care for residents.
- Education and Training: Conducting educational workshops and training sessions for staff, residents, and families on topics related to aging, mental health, caregiver support, and other relevant issues.
- Documentation and Reporting: Maintaining accurate and up-to-date records of assessments, interventions, and progress notes in accordance with regulatory requirements and professional standards.
Qualifications:
- Bachelor’s or Master’s degree in Social Work or a related field.
- Registration with the Irish Association of Social Workers (IASW) or eligibility for registration.
- Experience working with older adults and/or individuals with complex needs preferred.
- Strong interpersonal skills and the ability to communicate effectively with diverse populations.
- Empathy, compassion, and a genuine desire to improve the lives of others.
- Ability to work independently and as part of a multidisciplinary team.
